    吃这一类药，千万别喝热水！很多人都忽视了

    　　从小到大，几乎人人都吃过“胶囊”这类药物。服药时，一杯热水十分贴心，但在医生看来，“用热水送服胶囊”反而是个危险的操作。 　　《生命时报》邀请专家解读其中的原因，并提醒你哪些药物不宜热水送服。 吃胶囊药，不要用热水送服 　　上海交通大学医学院附属瑞金医院药剂科副主任石浩强介绍，胶囊是临床常见的药物剂型，约占医生开具处方的20%，但它有个弱点——怕热水。 　　根据胶囊“囊材”的不同，可分为明胶胶囊和植物胶囊。其中，明胶是应用最多的成囊材料，这是一种蛋白质制品，成分中85%~90%为蛋白质，不溶于冷水，但容易在温水和热水中溶解。 　　有试验发现，胶囊在50℃的水里浸泡10秒钟，就会变软、变黏稠。哪怕胶囊外壳只是化了一部分，也会变得像粘了一块泡泡糖一样，拉丝、发黏。 　　胶囊长时间和食管接触、在错误位置溶解、药物提前释放，可能会带来以下健康风险。 　　1.损伤食管 　　若胶囊粘在食管，可能在食管局部形成高渗环境，造成灼伤，甚至引发药物性食管炎。 　　尤其当胶囊内包裹的是刺激性较强、气味浓烈的药物时，如多西环素、利福平、布洛芬、双仑磷酸钠等，若外壳提前受损，更易刺激食管。 　　2.伤胃 　　有些胶囊是肠溶的，胶囊作为保护壳一路“护送”药物进入肠道，让药物成分避开胃酸分解，安全到达肠道才能被有效吸收。 　　如果因为水温过高破坏了结构，药物就无法按预期到达肠道发挥应有的作用，还可能对胃黏膜造成较强的刺激。 　　3.引发不良后果 　　有些胶囊属于缓释、控释型的，需要缓慢、平稳地释放，一旦外壳提前破坏或掰开服用，可能产生突释等不良反应。 　　石浩强提醒，尤其是软胶囊剂，药典要求存放温度是8℃~20℃。存放温度过高或用热水送服，容易导致粘连。 　　除了不宜用热水送服胶囊外，吃完胶囊类药物之后，最好过一会儿再喝热水。 建议用常温水，低头下咽 　　不可否认，并不是每个人都能轻松吞下一粒胶囊。据不完全统计，25个成人中就有1个人存在吞咽困难。石浩强表示，对于这部分人群，服用胶囊时可以尝试以下方法。 　　1.采用直立站位或保持上半身的直立。 　　2.在服用胶囊前，先喝一口水润一润嗓子，避免吞咽时胶囊粘在食道上，接着将胶囊放入口中，喝水将其吞下。 　　3.采用前倾法有助于顺利将胶囊吞下，其步骤是：首先将胶囊放在舌头上，然后喝一口水，不要马上咽下，低头，并让下巴向胸口倾斜，让胶囊浮到口腔后部，然后吞下。 　　4.送服胶囊时不宜用热水(稍低于40℃)，以免导致胶囊壳软化不利于吞咽。 　　5.用足量的水送服，以200~250毫升为宜。 这些药也不适合热水送服 　　正常人体口腔温度在36.2℃~37.2℃，低于这个温度为“凉水”，接近这个温度为“温水”，高于这个温度为“热水”。 　　多数情况下，服药时宜用温水或凉白开送服，即40℃的水，不要超过60℃。 　　除了胶囊外，以下药物也不宜热水送服。 　　维生素类 　　如维生素C、B族维生素，它们遇热后容易被氧化分解失去药效。 　　助消化类 　　如胃蛋白酶合剂、胰蛋白酶、多酶片、酵母片等，都含有助消化的酶类。酶是一种活性蛋白质，遇热后会凝固变性。 　　含活性菌类的药物 　　如复方乳酸菌营养剂、双歧杆菌三联活菌胶囊。根据说明书，活菌制剂要冷藏保存，用凉、温水冲服。 　　止咳糖浆类 　　包括急支糖浆、复方甘草合剂、蜜炼川贝枇杷膏等，它们是将药物有效成分溶于糖浆，或用浸膏配置而成。 　　服用后，糖浆覆盖在病变的咽部黏膜表面，形成一层保护膜，便于快速控制咳嗽，缓解症状。若用热水冲服，会降低糖浆的黏稠度，影响疗效。此外，喝完糖浆5分钟内也不宜喝水。 　　(生命时报微信公众号) 【编辑:张令旗】|dfg|
